The Patriot Support Program at Coastal Harbor Health can be ideal for these individuals as we provide the opportunity to carefully identify and address the difficulties they are facing. Our military mental health team is staffed by veteran and retired military personnel who understand military culture. Since veterans remain under the care of the VA and other psychiatric care facilities after discharge, we communicate thoroughly with VA case managers and care coordinators to help ensure that these patients transition safely to their homes or designated placements.
